Human Research Determination

1. Complete the Human Research Determination form.  This can be found at http://orcr.arizona.edu/hspp. This form will help determine whether the project meets the federal definition of research involving human subjects.
   
   
2. Submit the completed form and protocol via e-mail to Alice Pasvogel (Office of Nursing Research) at pasvogel@nursing.arizona.edu.
   
   
The Principal Investigator will be notified if additional information is needed.


3. The Office of Nursing Research will send the material to the College of Nursing Departmental Review Committee.
   
   
4. The College of Nursing Departmental Review Committee will review the material. The Principal Investigator will be notified if there are any concerns or if additional information is needed. 
   
   
5. The Office of Nursing Research will obtain the signature of the Scientific/Scholarly Review and a copy will be sent to the Principal Investigator.
